Psychiatric Case Management (PCM)

Psychiatric Case Management (PCM) is a case management program for Medicaid-eligible children and adults. Psychiatric Case Management provides services in the home, community, and school–and serves as an extra support to outpatient counseling and medication management services. We strive to assist our clients in becoming as independent as possible.

PsyCare uses a recovery-based model that encourages personal responsibility and development of skills to help the client take control of his or her life, rather than depending upon systems and agencies. We are a client-driven program focused on helping you attain your goals. We believe in a workable partnership between the client and our case workers. PCM is available in all seven locations in Mahoning, Trumbull, and Columbiana counties.

PCM for Adults

For adults, case management:

  • can help with accessing community resources and benefits like Social Security, food banks, housing, transportation services, and government agencies.
  • provide assistance in finding medical, social, educational, spiritual, and other services needed.
  • provide behavioral and emotional support for managing mental health symptoms, as well as parenting support for those with children experiencing emotional difficulties.
  • link clients with courts, government meetings, and specialist appointments.

We are strength-based and believe that the development of a positive self-image and a stable living situation is critical to long-term symptom management and personal happiness. An individual is assigned a PCM staff member who is knowledgeable and has skills in mental health treatment. The client works directly with the case worker to identify personal goals and community resources.

PCM for Children

PsyCare has case management services for children with difficult-to-treat emotional disorders and/or developmental disabilities. PCM can assist with assessing the child’s and family’s needs and find resources required to address those needs. We work closely with schools and families to make sure your child has all the supports in place to help him or her become a successful student. We attend school meetings with you and your child, and assist with IEP development and advocacy based upon your child’s educational needs. A specific and individualized service plan is developed to identify your child’s strengths and areas of concern.

PCM monitors the child’s progress at home and at school. We help parents develop specific behavior plans to reduce problem behaviors at home and school. PsyCare works closely with specialists like neurologists, occupational therapists, physical therapists, counselors, and medical specialists to make sure your child has the best and most complete treatment team possible. PCM staff can provide help to other children in your home if they are clients of the agency.
